Welcome to the latest of our weekly newsletters. We regard it as a priority that all patients are kept fully informed of developments at our Adlington, Croston, Buckshaw, Eaves Lane, Chorley site and Lostock Hall surgery sites. We hope these regular updates continue to prove useful.
Temporary Surgery Closure Next Week
Essential training which is being delivered to NHS staff nationwide is taking place next Tuesday afternoon, June 11.
Please note on this date, all our sirgery sites will close at 12.30pm and all phone lines will be closed from this time.
During this period, please call 111 or if the issue is an emergency, please call 999.
Our normal enhanced service wil operate from 6.30pm that day and phone lines at all our sites reopen at 8am the following day, Wednesday, June 12, with doors open 8.30am.
New Patients
We are currently accepting new patients at all our surgery sites.
If you know anyone who lives in our practice area and would like to register, please ask them to fill in a registration form.
To register, all you need to do is complete our online form by clicking here.
When you have completed all of the details, click the ‘Send’ button.
We will then contact you to attend the practice for a New Patient Health Check.
If you do not have internet access please call into the surgery for a hard copy.
Please note the first time you visit the surgery, we will request that you sign a hard copy of your registration form to complete your registration.
Holiday Vaccinations
If you are going abroad in the peak holiday season of July and August and think you may require vaccinations, please act now as you must submit a request form at least eight weeks in advance of your departure date to allow us to make appropriate arrangements.
You can access the form on our website at www.nmhealthinnovations.co.uk
Just click the tab marked ‘Services’ on the bar at the top of the home page, then choose ‘Clinics and Services’ and scroll to the bottom of the list where you then click on ‘Travel Vaccination and Travel Advice.’
Please then complete the online form and click the ‘submit’ button.
If you have no internet access, please pick up a hard copy from your surgery & drop off at reception.
When the form is received, one of our nurses will get back to you to advise if vaccinations are needed and if so, which ones, and will make an appointment for you.
Chorley Homeless Group and Chorley Support for Ukraine Appeals
Thank you for your recent contributions which have been delivered to the groups and have been gratefully received.
We are continuing to accept donations so if you’ve any toiletries, sanitary products, baby items, pet food, non-perishable food items, shoes, clean duvets, blankets, towels, coats or sleeping bags, please drop them off at any of our surgeries during opening hours.
Please mark your bags if you have a preference for which appeal you wish them to go to.
