Dr Muttucumuaru’s funeral
We would like to thank everyone who attended Dr Muttucumaru’s funeral at Fulwood Free Methodist Church last Friday.
It was a very moving and the attendance and tributes offered gave an indication of the deep respect and love in which he was held.
We would also like to thank our patients for their patience and understanding with regards to the temporary closure of Lostock Hall Surgery, Eaves Lane Surgery and Croston Medical Centre on the morning of the funeral to enable our staff to attend.
Type 2 Diabetes Prevention Week
This week is Type 2 Diabetes Prevention Week which is running up to this Sunday, May 31.
This disease can impact your health significantly if left untreated and millions of people in the UK don't know they are at risk of developing it.

Holiday Vaccinations
The holiday season is almost upon us and if you require vaccinations for your trip abroad, please make sure you think ahead.
You need to act now as you are advised to submit a request form at least eight weeks in advance of your departure date to allow us to make the appropriate arrangements.
You can access the form on our website at www.nmhealthinnovations.co.uk
Just click the tab marked ‘Services’ on the bar at the top of the home page, then choose ‘Clinics and Services’ and scroll to the bottom of the list where you then click on ‘Travel Vaccination and Travel Advice.’
Please then complete the online form and click the ‘submit’ button.
If you have no internet access, please pick up a hard copy from your surgery & drop off at reception.
When the form is received, one of our nurses will get back to you to advise if vaccinations are needed and if so, which ones, and will make an appointment for you.
Chorley Support for Ukraine Appeal
Thank you for your recent contributions which have been delivered to the Chorley Support for Ukraine depot and have been gratefully received.
We are continuing to accept donations so if you’ve any toiletries, sanitary products, baby items, pet food, non-perishable food items, shoes, clean duvets, blankets, towels, coats or sleeping bags, please drop them off at any of our surgeries during opening hours and we will deliver to the depot for you.
