Welcome to the latest of our weekly newsletters. We regard it as a priority that all patients at our Adlington, Buckshaw, Croston, Eaves Lane Chorley and Lostock Hall surgeries are kept fully informed of news relevant to them. We hope these updates continue to prove useful.
New Appointments System
The new triage process for booking appointments at your GP practice is now up and running as from yesterday, Wednesday, October 1.
If patients have not already done so, we would advise you download the NHS App — it's the easiest way to manage appointments, prescriptions and more.
If you have internet access, you can do so via this link www.nhs.uk/nhs-app.
Please note if you do not have internet access, you can still call the surgery and our reception staff will complete the triage form over the phone and submit it for you.
They can also answer any queries of concerns you may have.
Please note this change has been introduced by the NHS and the Government, not by our GP practice.
IMPORTANT - Proof of Address letters
All patients should note that we do not provide proof of address letters.
GP surgeries are medical services only and we cannot provide proof of address for:
• Banks
• Housing
• Benefits
• Any other purpose
If you are an asylum seeker, please contact SERCO or your housing provider for proof of address.

Temporary Surgery Closure
Please be advised that essential training which is being delivered to NHS staff nationwide is taking place next Tuesday, October 7.
On this date all our surgery sites will close at 12.30pm on the day and all phone lines will also be closed from this time.
With the exception of evening appointments with our enhanced service, there are no pre-booked appointments on that afternoon.
All surgeries will reopen as normal the following day, Wednesday, October 8, with phone lines open at 8am and doors open at the usual time of 8.30am.

We are backing the NHS’s annual Stoptober quit smoking campaign.
If you have internet access, search ‘NHS quit smoking’ to access the online resources.
Local support is available and sessions offering advice and resources are running from noon to 2pm every Wednesday up to October 29 at Duke Street Family Hub, Chorley PR7 3DU.
